It is the responsibility of the mother, father, or a person who has full knowledge of the facts of the stillbirth to complete a registration of stillbirth form. All stillbirths in Alberta must be registered. Once a stillbirth form is completed it is best to leave it at the hospital to be forwarded to Vital Statistics but under very special situations it can be mailed directly to Vital Statistics. The registering of a stillbirth document should be mailed to the Vital Statistics office within 10 days from the date of the event. The Registration of a stillbirth is a permanent document and it is required and very important that is completed properly and completed fully. Once the documents are completed but need corrections and an amendment is requested within 90 days of the event there is no fee. But if the amendment is requested after 90 days of the event then there will be a $20 government fee.
